K353 Sprinkler System – Maintenance and Testing

Automatic sprinkler and standpipe systems are inspected, tested, and maintained in accordance with NFPA 25, Standard for the Inspection, Testing, and Maintaining of Water-based Fire Protection Systems. Records of system design, maintenance, inspection and testing are maintained in a secure location and readily available.

CMS's Oct 2026 release lists 3,323 nursing homes cited under K353 in inspections from Oct 2025 to Sep 2026. 69% of those homes had also been cited under K353 at an earlier inspection (2,303 of 3,323).

Fire alarms · NFPA 72

K345 Fire Alarm System – Testing and Maintenance

A fire alarm system is tested and maintained in accordance with an approved program complying with the requirements of NFPA 70, National Electric Code, and NFPA 72, National Fire Alarm and Signaling Code. Records of system acceptance, maintenance and testing are readily available.

CMS's Oct 2026 release lists 1,868 nursing homes cited under K345 in inspections from Oct 2025 to Sep 2026. 49% of those homes had also been cited under K345 at an earlier inspection (906 of 1,868).

A home cited under both tags is counted once: 4,057 homes in all. All three trade tags, with state counts: CMS K-tags.

After the cited homes, a list adds homes whose last standard fire-safety inspection on CMS's file is old, oldest first. An old date can mean CMS hasn't loaded a newer one.

What a citation does and doesn't mean

It tells you

  • A state inspector cited the home under that tag at that inspection, with the scope and severity shown.
  • The status and correction date, as CMS lists them. 95% of the 5,227 K353 and K345 citations from Oct 2025 to Sep 2026 say "Provider has date of correction": the home reports it fixed.
  • "Provider has plan of correction" means a plan to fix it has been made. CMS's file doesn't list it as corrected.
  • Both tags cover records too. CMS-2786R asks for test and maintenance records that are "readily available", so a citation can be about paperwork.

It doesn't tell you

  • That the sprinklers or the alarm stopped working.
  • Who tests them today. The form asks Who provided system test, but CMS doesn't publish that answer.
  • That the file is complete. A federal audit (OIG A-09-20-02007) found fire-safety citations missing from Care Compare.
  • That it is up to date. In CMS's Oct 2026 release, the last 1–3 months of inspections are only partly there. The newest is dated Sep 23, 2026.

How to use it

Open with the home's next NFPA 25 or NFPA 72 inspection, or an annual inspection and testing contract. For example: "We run NFPA 25 and 72 inspections for homes near you. When is your next annual one?" If the citation comes up, say "cited" and give the status as CMS lists it. A home cited under the same tag before is the natural fit for an annual contract. Using citations responsibly

Questions buyers ask

Where does the data come from?

From CMS's public nursing-home files: Fire Safety Deficiencies, Provider Information, Inspection Dates and Ownership, plus the U.S. Census geocoder for map points. Every row links to its CMS record.

Where the data comes from

How fresh is it?

CMS publishes once a month. The current file was released Oct 28, 2026, covers inspections up to Sep 23, 2026, next file due Dec 2, 2026. Most inspections take 1–3 months to show up in CMS's file.

How we count: data sources

Do I get contact names and emails?

No. You get each home's name, legal business name, address, main phone, beds and chain, as CMS lists them. The main phone is the only contact CMS publishes, and there are no emails.

What counts as "near me"?

A straight-line radius from your shop, 60 miles by default. A home is held back, not guessed, when its map point is too close to call: within 1 mile of the edge, because CMS rounds its map points, or when CMS's point and the Census geocoder's point fall on different sides of the edge. Held homes are not listed, and the count then says "at least".

For some homes CMS gives only the center of the ZIP code. We look up the street address with the U.S. Census geocoder. When that doesn't place it, the card says "distance not stated", and the home is held back if it is within 15 miles of the edge.

Distances are "about N mi" in a straight line, not driving distance.

Do you cover hospitals or assisted living?

No. Only Medicare/Medicaid-certified nursing homes are in CMS's file. When CMS's hospital file lists the same street address, the card says so, because the generator and fire systems there may be the hospital's.

Will a competitor get the same list?

Yes. The records are public, so a competitor can get the same list. You pay for the finding, checking and mapping, every month.

What does it cost?

$249/month per 60-mile territory, per trade. Sprinkler + alarm is one trade, so one territory covers both K353 and K345. Founding firms: $149/month, locked for 12 months, for the first 20 firms. 14 days free, no card.
